Whenever a disease or a bad health conditions lasts longer than expected, the patient needs to keep the medical records safe and easy accessible. But whenever the symptoms get to look cured for a short or long span, people stops caring about the med records and and struggles to find the old records while diagnosed by the same health issues.

Based on the information architecture above, after few sketching of different screens I moved into wireframes, which will provide better quality of under standing for users. I started to create wire frames for all major screens in the workflows. such as Document screen, Upload document screen, Profile screen, Add new profile screen, edit Profile screen etc., which are the major screens in User flows.

After two rounds of User testing, one with wireframe prototype and one with high-fidelity prototype, I got some user feedbacks to incorporate in the design update process. Few notable changes are adding more details in the document cards, options to edit the document during the upload process and to make the ui looks more interesting with some new features.
